CR Community Health Services is a Providence-based organization dedicated to improving the health and well-being of Rhode Island families and adults through compassionate, community-centered care. Founded by Carlos Ruiz, a Licensed Community Health Worker with over 15 years of experience in human services and behavioral health, CRCHS provides wraparound support that bridges the gap between healthcare systems and the people they serve.
In partnership with Derrell Rose, LCSW, of WooSource Community Partnership, our team delivers Medicaid-approved, evidence-based services that address medical, behavioral, and social needs holistically. From health education and system navigation to housing coordination and wellness coaching, we empower clients to achieve stability, self-advocacy, and lasting wellness.

SERVICES
Our Community Health Workers guide clients through the healthcare system — from scheduling appointments to understanding insurance benefits.
Example: Helping a client connect with a primary care doctor, refill medications, and coordinate specialist referrals.
We provide one-on-one education and coaching to help individuals manage chronic conditions and make informed lifestyle choices.
Example: Teaching diabetes self-care and creating an action plan for improved nutrition and exercise.
We connect individuals with licensed behavioral health professionals for counseling, therapy, and addiction recovery services.
Example: Assisting a client in finding a therapist through Woo Source or other community partners while providing ongoing CHW check-ins.
We help clients access safe housing, food assistance, transportation, and other essential community resources.
Example: Working with Crossroads RI and Amos House to secure emergency shelter and apply for rental assistance.
Our CHWs address barriers such as unemployment, lack of childcare, or limited access to healthy food.
Example: Connecting a parent to job training programs and the Rhode Island Food Bank’s family meal program.
We ensure clients leaving hospitals, shelters, or respite care receive coordinated follow-up and continued community support.
Example: Supporting a recently discharged patient by arranging home visits, medication reminders, and follow-up appointments.
